Accident and Emergency medicine, often called emergency medicine offers outpatient care for the treatment of acute and chronic illness and injury. A & E doctors are generalists, in the broadest sense of the term, who specialise in resuscitation, injuries to the body, serious illnesses such as CPD for example

Anaesthesia

Anaesthesia is an acute medical specialty which is about total patient care. It combines an in-depth understanding of applied physiology and pharmacology with the need to be practically adept. Multi anaesthetic use on the same patient is now widespread – for example, for a total hip replacement (THR) it is current practice to have:

  • A local anaesthetic around where the incision will be made
  • An epidural or spinal block to completely numb all of the lower body
  • A general anaesthetic (GA) as well. Because of the first two ‘locals’ the strength of the GA is reduced and the patient recovers much quicker.

As intra-operative care includes continual observation and monitoring of the patients, attention to detail is important, and anaesthetists must have an ability to respond appropriately to sudden changes. Similarly, it follows that there is a transparent need to maintain good resuscitation skills both for peri-operative patients and emergency cases, often in acutely ill patients.

Sub-specialisation within the theatre sessions is increasing and includes cardiac, thoracic, neuro, plastics, paediatrics, obstetrics and trauma and emergency surgery. Intensive care medicine and pain management are two other major sub-specialty areas, both of which involve a significant amount of work outside the theatre environment and also more prolonged patient care.

Because most hospitals have largish anaesthetic staff numbers, on-call rotas are generally good – one in six upwards

Obstetrics & Gynaecology

Doctors and consultants working in obstetrics and gynaecology are concerned with the care of the pregnant woman, her unborn child and the management of diseases specific to women. Most consultants are generalists but the specialty offers a wide range of sub-specialties, which include gynaecologic oncology; gynaecological urology; reproductive medicine; community gynaecology and materno-fetal medicine.

Both obstetrics & gynaecology specialties include a large amount of hands on procedures as the surgical work is varied and involves close co-operation with other specialties such as urology, colorectal surgery and oncology. Other medical aspects of the specialty involve liaison with endocrinologists, renal physicians and cardiologists and there are many opportunities available to work closely with the community, both in obstetrics and gynaecology.
